Total Care for the Homeless Coalition

A regional Continuum of Care website connecting people in a housing crisis with coordinated entry while publishing the planning, funding, data, and governance resources used across thirteen South Carolina counties.

The need

TCHC must serve two very different needs: give a person in crisis a clear first step toward housing assistance, and provide member agencies with current HMIS, coordinated-entry, PIT count, YHDP, funding, policy, and public-notice materials.

The approach

The website prioritizes coordinated-entry assessment and access points while organizing technical programs, membership, governance, news, events, PIT data, NOFO competitions, downloadable applications, timelines, policies, and support forms for the regional provider network.
